How do I get a replacement card or a certification form  if I lose mine ?

We do not keep copies of forms  and cards at our office. Duplicate forms need to be reproduced upon request.

You will need to submit a written request  to have duplicate cards and forms issued.  There is a $8.00 replacement fee per class.  Submit your request with your full name, current address, a phone number to contact you at if we have any questions,  and the $8 fee (cash or check) .  After we verify your class information, replacement paperwork will be mailed within a couple days to the address you provide.

Requests/payments should  be mailed to Central Ohio CPR, 6260 Huntley Road, Columbus, OH  43229.
