How do I get a replacement card or certifciation form  if I lose mine ?

We do not keep copies at our office, duplicate forms need to be reproduced upon request.

You will need to submit a written request  to have duplicate cards and forms issued.  There is a $8.00 replacement fee per class.  Submit your request with your full name, current address, a phone number to contact you at if we have any questions,  and the $8 fee (cash or check) .  After we verify your class information, replacement paperwork will be mailed within a couple days to the address you provide.

Requests/payments should  be mailed to Central Ohio CPR, 6260 Huntley Road, Columbus, OH  43229.

I’m in childcare and my certification expired.  Can I take a recert class ?

CPR and BFA classes are all the same whether you are an initial or a recert.

Child Abuse and Communicable Disease recert classes may be taken up to 60 days past your expiration date.   If more than 60 days have pasted, you are required to take an initial class.

Do you offer any  recert classes  for CPR ?

We offer the same class whether you are new to CPR or have taken it many times, as the guidelines continually are changing.  This assures the most up-to-date information.

I took my HCP-BLS course on-line. Do you offer the skills testing ?

We do skills testing upon request.  Once you complete the course and have your printed certificate, contact us and we will setup that portion with  you.

The procedures are listed under the class description for Skills Test.

How do I cancel a class I registered for ?

We ask that you cancel at least 24 hours in advance so others may take your place.   If a class is full and closed as a result,   ½  the fee will be charged for no-show’s.  You may leave a phone message or an email to cancel a class.
